Passport is essential in life for foreign visits, every doctor must have it, but after joining government service it is very tough job to making a passport. In the end you would realize that medical college was right time to make passport because that time you did not need any NOC/PIL/Annexures.

But we here to help you so that all related format and Guidelines are here –
Fill PIL ANNEXURE ‘H’ and paste a good looking ‘Passport size’ photo on it and duly signed by your Officer In-charge (Hospital in charge/ BCMO/ PMO/ CMHO) and HOD in case of In-Service Resident.

Passport office doesn’t take any document from you, they only scan all the documents.

PIL – Prior intimation letter, in form of Annexure H, so now no NOC needed from department, you have to just intimate them only.

After filling Annexure H send one copy to Director Public Health by email as an intimation
